University of health sciences has announced admission for the undergraduate programs in its Kalashakaku campus located near motorway interchange GT Road Lahore 

FOLLOWING COURSES ARE BEING OFFERED. 

BS TRANSFUSION MEDICINE 4 YEARS 

BS PUBLIC HEALTH 4 YEARS

BS MOLECULAR AND GENETICS 4 YEARS

DOCTOR OF PHYSICAL THERAPY 5 YEARS

BS BIOMEDICAL SCIENCES 4 YEARS.

درخواست جمع کرانے کے لیے یونیورسٹی کی طرف سے درج ذیل اقدامات کا مشورہ دیا گیا ہے۔ 

مرحلہ 1: آن لائن درخواست کے لنک پر جائیں:

 مرحلہ 2 درخواست فارم کو مطلوبہ معلومات (ذاتی تفصیلات، قابلیت وغیرہ) کے ساتھ پُر کریں

۔مرحلہ 3: اپنی اہلیت کے مطابق پروگراموں کے لیے اپنی ترجیحات کا آرڈر درج کریں۔ 

مرحلہ 4: اپنی درخواست کے حتمی جمع کرانے سے پہلے، 2000 روپے کی پروسیسنگ فیس/ (نا قابل واپسی) بینک کے ذریعے جمع کروائیں

۔ چالان نیشنل بینک آف پاکستان، شیخ زید ہسپتال برانچ لاہور (A/C # 3164498127) یا جمع کرائیں۔

 اکاؤنٹ کے عنوان کے تحت یونیورسٹی کے آن لائن اکاؤنٹ (PK53NBPA1707003164498127) میں اتنی ہی رقم: "CE UHS"۔ ادا شدہ چالان/آن لائن لین دین کی سلپ کو آن لائن 

درخواست فارم پر منسلکہ کے طور پر اپ لوڈ کرنے کی ضرورت ہے

۔ مرحلہ5 : اسکین شدہ دستاویزات اپ لوڈ کریں (زیادہ سے زیادہ 

  1Mb سائز:   

مرحلہ6: بھری ہوئی معلومات اور منسلکات کی تصدیق کریں۔ 

 جمع ہونے کے بعد درخواست دہندہ اس میں ترمیم نہیں کر سکے گا

۔ مرحلہ 7: اپنا درخواست فارم حتمی جمع کروائیں۔ حتمی جمع کرانے کے بعد، ایک تصدیقی نوٹیفکیشن ہو جائے گا آپ کے ای میل یڈریس پر موصول ہوا۔ 

سلپ کو سنبھال کر رکھیں۔ ابھی اسے یونیورسٹی میں جمع کروانے کی ضرورت نہیں ہے۔

Step 1: Go to the online application link: https://forms.uhs.edu.pk/ugpadd

Step 2: Check your eligibility (If you have 50% or above marks in HSSC/equivalent exam, you can proceed with your

application otherwise not).

Step 3: Fill in the application form with required information (personal details, qualifications, etc.).

Step 4: On initial submission, a password will be generated and it will be sent on your E-mail address. You can now

Login with the help of this password.

Step 5: Enter your Order of Preference for the programs as per your eligibility.

Step 6: Before final submission of your application, deposit a processing fee of Rs.2000/ (Non-refundable) through bank

challan at National Bank of Pakistan, Shaikh Zayed Hospital Branch Lahore (A/C # 3164498127) OR deposit the

same amount in University’s online account (PK53NBPA1707003164498127) under Account Title: “CE UHS”.

Paid challan/online transaction slip is required to be uploaded as an attachment onto the online application form.

Step 7: Upload scanned Documents (Max.Size: 1 Mb/each document) as mentioned on the application form.

Step 8: Verify the filled information and attachments. Once the form is submitted the applicant will not be able to edit it.

Step 9: Make the Final Submission of your application form. After final submission, a confirmation notification will be Received on your E-mail address.
